H2HC Outreach (Homeless to Housing Coalition) provides support services to individuals experiencing homelessness and/or at risk of becoming homeless.

Outreach

• creates meaningful, trusting, professional relationships and sustains them.
• provides support in accessing and maintaining accommodation.
• increases community supports

Prevention

• offers support in reducing tenancy issues and maintaining a successful residency.
• mediates between landlord and tenant.

Information & Referrals

• provides information about government and community services available to assist.
